ILLINOIS CERTIFICATE OF RELIGIOUS EXEMPTION TO REQUIRED IMMUNIZATIONS AND/OR EXAMINATIONS FORM

Below is the form that must be completed by parents/guardians in order to be exempt from immunizations or examinations. Please complete and have the student's primary doctor sign the document. Once this is completed please return to the student's school.

NEW IMMUNIZATION REQUIREMENT FOR THE 2015-2016 SCHOOL YEAR

Meningitis Vaccine requirement for grades 6 and 12.

Attention Parents: A new vaccine is required for entry into grades six (6th grade) and twelve (12th grade) this coming school year, beginning August 2015. This vaccine protects students against a severe form of bacterial meningitis, as well as other diseases caused by this bacteria. Please make plans to assure that your student is protected against meningitis and ready to start school in the fall.

IMMUNIZATION

On or before December 1 of each year, the school district makes publicly available the immunization data they are required to submit to the State Board of Education by November 15.

On behalf of the Illinois State Board of Education, UChicago Impact will administer the third annual Illinois 5Essentials Survey, a unique opportunity for teachers, students, and parents to have a voice in improving their schools. From January 30 - March 29, 2024 teachers, students, and parents will be asked to take a 20-30 minute research-based survey to help identify strengths and weaknesses in their school’s learning environment.

This survey is based on the Five Essentials for School Success, an evidence-based framework developed by the University of Chicago. This framework identifies how school performance on five factors or “essentials” can indicate important student outcomes, including improved attendance and larger test score gains.

The 5Essentials areas are:

  • Effective Leaders

  • Collaborative Teachers

  • Involved Families

  • Supportive Environment

  • Ambitious Instruction
